I do qualify for transportation, now what?

Scheduling A Transport

Your, or the patient's, doctor must provide a Physician Certification Statement (PCS) and fill out the required forms within at least 7 business days prior to transport.


*Medicaid patients require paperwork to be sent off to get approval prior to transport. This process can take up to 7-14 business days.



What To Expect The Day Of Transport

Once a transport is scheduled, a fully equipped ambulance and two EMT's will arrive at least 30 minutes prior to the scheduled appointment time (arrival time will vary on location to and from). Two EMT's will load/unload and transfer you or your loved one to and from a bed to a stretcher. One EMT will ride next to the patient during transportation. One family member is allowed to go along with the patient only if the patient is under the age of 18, but we do ask that you sit in the front of the cab for safety reasons. The two EMT's will be on standby with the patient until the patient returns to origin from the appointment (only if the patient's condition is not life threatening)


*If the patient is having a treatment procedure or surgery, the patient will be dropped off and left in the care of the receiving nursing staff and picked up at a later time.
